Here are some info that you should provide when describing your injury, according to physiotherapy diagnosis. Not all may be appropriate, but just give what you know. This is just a rough guide and not exhaustive. This would give people a better understanding of your injury.
Disclaimer: I am not a qualified physiotherapist, so opinion may not be 100% correct. Other disciplines like chiropractic or osteopathy may have different diagnostic tests, so this method is not the only one available.
1) Where is the injury/pain?
2) Any numbess/weakness/pins and needles?
3) Any locking/giving way?
4) Is the pain continuous or on-and-off?
5) Pain level (0:no pain------10:max pain)
6) Deep or superficial?
7) Aggravating factor What kind of movements/activity aggravate the injury?
8) Relieving factor What kind of things relieve the injury eg: rest, ice, hot pack etc?
9) 24h behaviour Any change in pain/stiffness or any other things related to injury in the: morning, daytime, night/sleep?
10) Investigations Have you taken any x-ray or something like that?
11) Current history of injury How did you injure the bodypart? Any things extraordinary that you did? Did it come on gradually? Have you sought any treatments? etc.
12) Past medical history Have you injured the same part before? Nature of injury? Have you injured other bodyparts before? Nature of injury?
13) Social history Occupation, hobby/sport, general health etc. Any recent unexplained mass loss? Are you taking any medications?
That's the subjective tests which is used to interview patients with musculoskeletal injury.
I would not put the objective tests here as they would be too complex for laymen, but if you know something, just post it here.
